DISTRICT FORMS
Kindergarten through twelfth grade students residing in a participating district may apply to attend any other participating school district. Complete one application for each student.
The Sandusky School Wellness Program provides services to promote academic success by helping
students achieve their best possible physical and mental health status. A Registered Nurse and Clinical
Master Level Social Worker will be available to all Sandusky students age 3-21 who have a written
consent on file. There are no out of pocket fees for these services. If you would like your child to be
eligible for services please complete this health history and consent form. Please one form per child.
Please fill out this form if your child has any Dietary Accommodations.
Please fill-out this form for any school associated fundraisers for Internal Accounts and Clubs (including Club Sports). All Fundraiser Requests need to be approved by the Superintendent prior to the fundraiser.
This form should be completed and submitted to the Business Office at Sandusky Community Schools within two weeks of the completion of the fundraiser.

Please attach all supporting documentation (quotes, etc.). Requests will be given to the Superintendent. Your request will be forwarded to the Pack Alliance for review and consideration. All funding requests must be received 14 days prior to a Pack Alliance's regularly scheduled meeting (2nd Monday of the month).
SANDUSKY ELEMENTARY SCHOOL FORMS
Parent or Guardian acknowledge the Handbook was reviewed with student for the 2024-2025 School Year.
SANDUSKY JR./SR. HIGH SCHOOL FORMS
This form must be signed and returned by Friday, May 1st, to attend the 2026 SHS Prom. Student who wish to attend prom must be in attendance a full day of school the day before prom. For Seniors, this is a partial day as they dismiss early.

All guests planning to attend the 2026 SHS Prom must complete and submit this application to the SHS Principal’s Office by 3:00 PM on Friday, May 1. Guest passes will not be issued to anyone who has not submitted a completed application. Applicants may be required to meet with Mr. Palmer or Mrs. Kravitsky. The principal or their designee reserves the right to deny admission to any student or guest.
Guests must be under 21 years of age. A photo ID is required for any guest not currently enrolled in an in-person high school. All prom attendees must be present for the full school day prior to prom.
